CMS' RDS Center will respond to an e-mail with an e-mail reply or a phone call depending on the nature of the inquiry. If the inquiry is about a specific RDS Application, please include the following information in an e-mail:
- Your Name
- Your E-mail Address
- Your Phone Number
- Plan Sponsor Name
- Plan Sponsor ID
- Application ID
Please do not include any Protected Health Information (PHI), as defined in the Health Insurance Portability and Accountability Act (HIPAA), in the e-mail.
